Immunizations

All students need proof of immunizations to enter SJS.

Requirments for all children attending PRE K-

• 4 or more doses of DTaP
• 3 or more doses of polio vaccine
• 1 dose of MMR
• 1-4 doses of Hib vaccine
• 3 doses of Hepatitis B vaccine
• 1 dose of varicella vaccine or a physician-certified reliable history of chicken pox disease.

Requirments for all children entering  K-

• 5 or more doses of DTaP
• 4 or more doses of polio vaccine
• 2 dose of MMR
• 3 doses of Hepatitis B vaccine
• 1 dose of varicella vaccine or a physician-certified reliable history of chicken pox disease.

Requirments for all children entering 7th grade 

• 2 dose of MMR
• 3 doses of Hepatitis B vaccine
• 1 or 2 doses of varicella vaccine (1 dose if <13 years old;  2 doses if >13years old) or a physician-certified reliable history of chicken pox disease.
• 1 booster dose of Td or Tdap

All pediatricians are aware of these requirements.  Parents please be sure to get an updated record at your child's annual visit.
