Welcome back to school!

Please be sure to meet with me to discuss  any health concerns or issues which may affect your child during the school day.  I will be at school during drop off time Wednesday and Thursday of the first week of school and Monday and Tuesday of the next week.  My office is located on the lower level, convent side.  Please remember the policy for medication administration in school, medications include  EPI pens.  All medication to be given at school must be brought to the nurse by a parent. 

Medication Administration- Medications to be given during the school day require (a) physicians order, (b) parental consent  and (c) 30 day supply of medication.  **All this to be in place before any meds will be administered at school.

As we progress through the year  please continue to notify me of any changes in your child's health as a result of injury or illness. 

A brief summary of health information needed for the health office:

Physical Exam- prior to first entry and every 3 years after.  Annual physicals if participating in the basketball program.  Please remember to get copies at your office visits this summer.

Lead Poisoning Screening- prior to entering kindergarten

Preschool Vision Screening - done by primary care physician, upon entry into kindergarten

Immunizations - see web site for further info

Children with special health care needs - it is vital parents meet with me at the beginning of the school year to develop a plan.

Emergency planning - the Health Information and Consent form must be completed annually for each child enrolled in Saint Joseph School.  It may be printed directly from the web site.
